NOTE: Online reservation must be made 48 hours in advance.  Your reservation is not complete until you receive a confirmation number.  For immediate service, please call our office 718-241-4322 or toll free 888-317-5466.  If you prefer to send your reservation via fax, please fax your completed form to 718-209-2462.

The credit card holder MUST be present at the pick up location.  If you are paying for someone else, please print and fax your reservation with a clear (legible) copy of the front and back of the credit card and a photo ID (Example: Driver License) to  718-209-2462
